Preparing Community Health Workers (CHW) For a New Level of Engagement

• Virtual, in-person and internship learning opportunities to develop well-rounded CHWs who serve as front-line public health workers
• Assisting providers at all levels in addressing the social determinants of health and health Iinequities at the ground level through assessments and intervention, basic screenings and prevention and client advocacy
• Bringing practical, non-clinical skills, personal experience and helping hands to the care team to better serve community members where they are – and where they live
• Extending the care team in a variety of settings to accomplish community health goals in a collaborative, cost-effective manner with CHWs who have a strong connection to the communities they serve
• Providing support to governmental units and community-focused organizations on key administrative mandates such as Medicaid Renewal

Have You Been Screened for Colorectal Cancer?

Colorectal cancer is found in the colon or rectum. It is the second leading cancer killer in the United States when the numbers are combined for men and women. Screening can find precancerous polyps in the colon orrectum that can be removed before they have a chance to turn into cancer. Screening is also important to find existing colorectal cancer early when the treatment works the best.

Colorectal cancer screening recommendations from the American Cancer Society:

• Adults with an average risk start regular screening at age 45

  • Done with either a sensitive stool-based test that looks for signs of cancer in a person’s stool or with an exam that visually looks at the colon or rectum

  • People in good health with a life expectancy of more than 10 years should continue regular colorectal cancer screening through the age of 75

• Adults 76 to 85 years of age, the decision to be screened should be based on a person’s preferences, life expectancy, overall health and prior screening history

The Introduction to Team-Based Care course helps the learner better understand how to work in a multidisciplinary care team and in collaboration with the patient. Open to all members of the practice to gain foundational knowledge in Team-Based Care. Introduction to Team-Based Care will include:

• Why, What, Who and How: Team-Based Care
• Care Management Process
• Outcomes and Triple AIM
• Billing Applications

*This course is required for all care team members new to their role in order to bill PDCM codes.

The Patient Engagement’s goal is for all Care Team Members to learn engagement tools/skills in order to have productive conversations with patients about their health including basic motivational interviewing skills.

The Care Team Member will build upon this foundation, to utilize patient engagement skills in different situations such as Medication Assisted Treatment, (MAT) and Palliative Care.

The Foundational CM Codes and Billing Opportunities course builds upon the Introduction to Team-Based Care course, focusing on reimbursement for care management services. The course is designed to support and train physician organizations and practice staff on care management billing.

The course includes:

*This course is open for identified Physician Organization billing subject matter experts.

Learning Objectives:

• Define health care coding and health care billing
• Describe the two common health care coding classification systems
