Why PTI’s Community Health Worker (CHW) Training Program?

Practice Transformation Institute’s (PTI) Community Health Worker (CHW) Training Program has been recognized by the Michigan Department of Health and Human Services (MDHHS) as an approved Community Health Worker Training Program. This program has been designed with health care organization needs in mind. CHWs will become an important part of the practice team in supporting care management services. Participating in PTI’s program will allow CHWs to be eligible to bill for allowable services for Medicaid and Medicare, as well as BCBSM (in addition to completing Introduction to Team-Based Care).

• Focus on social determinants of health (SDOH) screening, assessment, and referral to appropriate community resources with follow up
• Emphasis on communication and motivational interviewing skills needed for effective patient interactions, including patient-centered goal-setting
• Comprehensive information regarding healthy lifestyles, prevention and screening, and common chronic conditions
• Concentration on professional and community capacity building, documentation, and assessment and interventions
• Attention to health inequities, cultural humility and competency, health literacy, advocacy, confidentiality, and legal and ethical issues
• Application of concepts learned through discussion, case studies, homework, and internship opportunities

Meeting Schedule: Twice a week for 9 weeks

Practice Transformation Institute is accredited by the International Accreditors for Continuing Education and Training (IACET) and offers IACET CEUs for its learning events that comply with the ANSI/IACET Continuing Education and Training Standard. IACET is recognized internationally as a standard development organization and accrediting body that promotes quality of continuing education and training.

Practice Transformation Institute is authorized by IACET to offer 0.6 CEUs for this program. (Each individual session is 0.6 CEUs)

Rethink Your Drinking in Alcohol Awareness Month

Springtime is a fun time for a lot of us. Vacations may increase with the kids off for Spring Break and we are all ready to blow past the winter with the fun of the warmer temperatures. Party days are ahead. But there may be some of us who need to pay more attention to how we use those party days.

April is Alcohol Awareness Month and this is the time to increase not only public awareness about alcohol but education regarding the treatment and the prevention of alcohol abuse. Alcohol abuse is a preventable disease and treatments for it are good.

The 2020-2025 Dietary Guidelines for Americans continues to recommend one drink or less a day for women and two drinks or less a day for men. Alcohol misuse or heavy drinking is described as four or more drinks on any day or eight or more per week for women; for men it is five or more drinks on any day or fifteen or more per week.

Alcohol Use Disorder (AUD) is defined as an impaired ability to stop or control alcohol use despite social, health, or occupational consequences. It can cause lasting changes in the brain and can make patients vulnerable to a relapse. Most people who have AUD benefit from treatment such as behavioral therapies, medications, or a combination of both.

The Introduction to Team-Based Care course helps the learner better understand how to work in a multidisciplinary care team and in collaboration with the patient. Open to all members of the practice to gain foundational knowledge in Team-Based Care. Introduction to Team-Based Care will include:

• Why, What, Who and How: Team-Based Care
• Care Management Process
• Outcomes and Triple AIM
• Billing Applications

*This course is required for all care team members new to their role in order to bill PDCM codes.

The Patient Engagement’s goal is for all Care Team Members to learn engagement tools/skills in order to have productive conversations with patients about their health including basic motivational interviewing skills.

The Care Team Member will build upon this foundation, to utilize patient engagement skills in different situations such as Medication Assisted Treatment, (MAT) and Palliative Care.

*It is strongly recommended to take Introduction to Team Based Care before taking this course.

The Foundational CM Codes and Billing Opportunities course builds upon the Introduction to Team-Based Care course, focusing on reimbursement for care management services. The course is designed to support and train physician organizations and practice staff on care management billing.

The course includes:

• Care Team Coding and Billing is a Team Sport
• Billing Optimization
• Billing Applications
• Billing Resources

*This course is open for identified Physician Organization billing subject matter experts.

Learning Objectives:

Members of the health care team; Nurses, Social Workers, Pharmacists, Registered Dietitians, Medical Assistants, Community Health Workers, and others will be able to:

• Identify members of a care team
• Define health care coding and health care billing
• Describe the two common health care coding classification systems
• Explain the importance of documenting the encounter
• Illustrate how a care team member may impact risk adjustment and the financial model of a practice
