Practice Transformation Institute Offers Opportunities to Receive Program Leader Certification Through Personal Action Toward Health (PATH) Workshops

Troy, Mich. —Dec. 26, 2014—Practice Transformation Institute (PTI), providing the health care community with continuing medical education and customized learning programs that improve patient health outcomes and the individual care delivery experience, is offering the Personal Action Toward Health (PATH) workshop series to community members at no charge beginning Jan. 8 in Madison Heights.
PATH is Michigan’s version of the acclaimed Stanford Chronic Disease Management Program. The four-day training provides certification as a Program Leader in the Chronic Disease Self-Management Program and offers tips on how to facilitate the program, focusing on the following key areas:

  • Managing symptoms
  • Communicating with health care professionals
  • Problem solving
  • Setting weekly action plans
  • Dealing with anger, fear, pain, frustration and depression
