Personal Action Toward Health (PATH) Webinar Will Introduce Physicians to Self-Management Education Program Aimed at Healthy Living for Patients with Chronic Conditions

Troy, Mich. —April 18, 2016—Practice Transformation Institute (PTI), providing the health care community with continuing medical education and customized learning programs that improve patient health outcomes and the individual care delivery experience, is partnering with MPRO to present a webinar on May 17 geared to physicians and focusing on Michigan’s Personal Action Toward Health (PATH) workshop series. PATH is based on Stanford University’s Diabetes Self-Management Program (DSMP) and is an evidence-based program that provides multiple benefits to patients.
The complimentary webinar will be conducted by MPRO colleagues, Andrea Lauhoff, MHSA, senior clinical quality consultant, and Mark Loush, LMSW, CAAC, clinical quality consultant. It will be held on Tuesday, May 17 from noon to 1:00 p.m. Register online at:
PATH is a six-week workshop that offers tips on day-to-day living to help participants improve their health, feel better andbe better able to face the challenges of living with an ongoing health condition. The workshop focuses onthe following key areas:
• Dealing with not feeling well
• Talking with health care providers, friends and family members
• Relaxing and overcoming stress
• Increasing energy levels
• Accomplishing everyday tasks more easily
• Staying independent
The webinar objectives for physician participants include:
• Define evidence-based programs and their benefits to patients
• Hear testimonials from past participants
• Learn how PATH can benefit patients in managing their chronic conditions
• Identify how the physician’s practice or organization can get involved with PATH
