New PTI Program Trains Pharmacy Professionals in Care Management

Troy, Mich. —Dec. 29, 2015—Practice Transformation Institute (PTI), providing the health care community with continuing medical education and customized learning programs that improve patient health outcomes, announces a care management training program specifically designed to integrate doctors of pharmacy, registered pharmacists and pharmacy technicians into the inter-professional healthcare team. PTI recently trained nearly 110 members of the pharmacy community, according to PTI Director of Operations, Harmony Kinkle.
“Pharmacists are increasingly performing more patient care activities such as health coaching and medical therapy management in a variety of settings and spending less time in the traditional role of dispensing medications,” Kinkle said. “Simultaneously, the pharmacist’s role is expanding further to provide leadership in quality improvement initiatives and medication-related measures. PTI’s pharmacy care management program recognizes the pharmacy community’s evolving role in team-based care and as a profession.”
PTI’s pharmacy care management coursework provides members of the pharmacy community with core competencies to become members of high-functioning, patient-centered teams focused on promoting health improvement, wellness, quality and disease prevention. The coursework is comprised of a blend of online courses and in-person training. Topics covered in the training include:

  • Introduction to Care Management Principles
  • Introduction to the Patient Centered Medical Home
  • Integration of the Community Pharmacist into the Practice Team
  • Linkage to Community Services
  • MQIC Evidence Based Guidelines for Diabetes, Heart Failure and Hypertension
  • Patient Registry
  • Group Visits
  • Admissions, discharge and transfer (ADT) activities
  • HEDIS Scoring
  • Behavioral Health

“Pharmacists can affect the quality of patient care, increase access to care for patients, provide cost-effective care and also quality oversight,” Kinkle said. “We are pleased to be providing additional training to the pharmacy community as they become an even more integral part of the interprofessional healthcare team.”
