Purpose:
Bring together physicians, administrators, office managers, business leaders and health care professionals to discuss the Patient-Centered Medical Home and its impact on health care in Michigan and other states. This program will address PCMH and public policy, payment and medical practice transformation.
Objectives:
- Outline the principles of Patient-Centered Medical Home
- Recognize the impact of a multi-payer demonstration projects in various states
- Explain the advantages of buying care that reflects the Patient-Centered Medical Home identify action steps purchasers can take to advance the PCMH concept
- Describe population-based outcomes including ways to maximize patient adherence to a customized self-care management program
- Illustrate the implications the Patient-Centered Medical Home concept would have on public policy, payment and provider reimbursement systems
