Intended Audience: This program is intended to bring physician leaders, residents, health care professionals, allied health professionals and medical practice managers together to discuss for each program.
 
Purpose:
 
Discuss the knowledge and tools necessary to integrate Individual Care Management into their clinical practice strategies. This content provides essential information to support the need for patients to receive organized, planned care that empowers them to take greater responsibility for their health which should improve their overall health status. Integrating a clear planning process prior to a patient’s visit enables the identification of gaps in care that exist and prepare the practice to develop strategies to address each of these gaps. The integration of individual care management strategies requires a transformation of care processes, staff responsibilities, the inclusion multidisciplinary teams, how information is accessed, documented and tracked and the expectations of patient.  
 
Content and Objectives:
 
Planned Care Visits: Identifying the Gaps Before Each Visit
· Define planned care
· Describe strategies for planned care visits
· Discuss the impact of using multidisciplinary teams to deliver care 
· Identify gaps in care before a patient’s appointment
 
Group Visits: Safety in Numbers…Learning From Each Other
· Explain what group visits are
· Describe planning strategies for group visits
· Identify how to implement group visits
· Discuss the impact of peer to peer learning on patient’s outcomes
 
Health Literacy
· Identify ways to assess a patient’s health literacy
· Discuss the impact of health literacy on patient’s outcomes
 
Test Tracking, Prevention and Follow Up
· Describe strategies for implementing a reliable system of test tracking and monitoring tests
· Explain processes for test tracking
