Intended Audience: This program is intended to bring physician leaders, residents, health care professionals, allied health professionals and medical practice managers together to discuss for each program.
Purpose:
Discuss the knowledge and tools necessary to integrate Self Management into their clinical practice strategies. This content provides essential information to support the understanding of the uniqueness of each generational group, the use of motivational interviewing to identify what is important to the patient in their change journey, integration of self management into the patient provider collaborative partnership, strategies to document self management using technology and the role of a health coach in practices. These content areas provide the necessary framework for building a patient centered medical home and transforming health care organizations.
Content and Objectives:
Generational Diversity and Its Impact on Self Management
· Identify the four generational groups
· Describe the characteristics of each generation
· Incorporate generational differences in daily communication strategies
Motivational Interviewing
· Describe the change process
· Identify the principles of Motivational Interviewing
· Discuss Motivational Interviewing approaches in various settings
Integrating Technology into Self Management
· Describe self management
· Identify various self management training programs
· Explain the use of technology to support self management
· Identify the role of action planning in self management
Health Coaching
· Describe the history of health coaching
· Explain the role of a health coach in the PCMH
· Relate health coaching and self management
