Implementation of the patient centered medical home (PCMH) requires primary care physicians and their practice teams to undergo a comprehensive practice transformation of their medical practices, from technology and office staff roles to new approaches to patient care. Under the Medical Practice Coaches Network (MPCN) umbrella, Practice Transformation Institute offers a coaches’ learning series and support to provide practice transformation coaching and mentoring services to physicians and their staff.
 
The Medical Practice Coaches Network (MPCN) is the only coaching forum of its kind in Michigan. Short-term commitment to MPCN results in long-term transformational change. That’s because the MPCN approach is based on a rapid transformation model to enable medical practices to quickly see the results of their coaching efforts.
 
The Medical Practice Coaches Network employs an intensive training approach that is new to private practice physicians.  Highlights include:
 
·         Field-based learning in the medical practice
·         Mentoring physicians and their practice teams over the course of a limited time period with the goal of transforming the practice into a Patient Centered Medical Home
·         Increasing the number and speed of practice transformations, with an end result of enhanced practice efficiencies and marked improvement in the quality of the patients’ experience
·         Using professional coaching and mentoring techniques, along with collaborative learning, to create physician leaders
·         Creating practice champions to assist the physician in sustaining long-term, positive change within the practice
