Medical Practice CME Programs from Start to Finish
The new 2010 CME mandates are quality based; learning doesn’t stop with the accrual of credits. There’s a new level of scrutiny that requires medical practices to perform a needs assessment, set a baseline of their patient populations, complete CME coursework and implement and then measure what has been learned. It is a daunting process for physicians to attempt alone, especially in busy medical practices with limited resources and administrative support. Practice Transformation Institute can make an immediate impact on your CME compliance with the following services:
·         A broad curriculum of accredited CME courses
·         Establishing population based registries to track patient data and measure CME outcomes against this baseline
·         Auditing CME compliance requirements
·         Measuring applied CME outcomes and providing and implementing improvement strategies
Practice Transformation Institute Can Guide Your Transition to the Patient Centered Medical Home

With its focus on a primary care and the creation of partnerships among individual patients, their personal physicians and patients’ family members, the Patient-Centered Medical Home (PCMH) is a progressive model of care developed and advanced by leading professional medical associations and agencies and destined to reshape the future delivery of health care at the practice level.

PTI provides targeted, measurable action-oriented training, mainly at The Physician Training Center in Madison Heights, to help medical practices implement the principles of PCMH. Our PCMH Foundation workshops transform a primary care practice into a Patient-Centered Medical Home, while our customized workshops improve and enhance practice settings. Learning modules focus on:

·         Establishing and using population-based registries
·         Making cultural and organizational changes
·         Enhancing operational efficiencies
·         Human resource policies, practices and procedures
·         Team building approaches that work
